If you hold a DVA Gold or White Card, you may be eligible for fully funded treatment through our services.

How to Get Started: 

Speak with your GP about a referral to Exercise Physiology. In most cases, this will be a D904 referral under the DVA Treatment Cycle.

To make things easier, we’ve included a downloadable referral form below in the Resources section — simply print it out and take it with you to your GP appointment (they’ll appreciate it).
